Diverging paths in physical activity and sports participation from adolescence to emerging adulthood: the Health Promoting Sports Club cohort study (HPSC3)

Summary
Longitudinal studies consistently demonstrate that physical activity (PA) declines from adolescence to early adulthood. Although PA declines on an average, some subgroups may increase PA while others maintain (in)activity. At the same time, this period of life, is one of the fundamental transition times in life. So far, there is lack of information on longitudinal objectively measured PA patterns of the given age group and the associated determinants and health outcomes. The aim in this study is to identify PA, sedentary behaviour (SB) and sports club participation patterns from adolescence to early adulthood (15-19-23 y). Moreover, the aim is to study determinants of above-mentioned change patterns related to life events, social and physical environment, health behaviour, health, diseases and injuries – plus health outcomes of these distinct patterns. Emerging adulthood as a study target period and longitudinal, objectively measured activity data of PA change are repeatedly highlighted as areas of PA research that needs further studies.
This prospective cohort study adds a new measurement wave (participants 22/23 year-olds) to previously (years 2013/2014 and 2017/2018) collected data of Finnish young people at ages 15 and 19. When the study started in 2013-14, the participants were recruited through schools (n=100) and sports clubs (n=156) from the six largest cities in Finland, plus surrounding communities. At the age 19, in total 649 (43% of baseline) young adults filled in questionnaire on health behaviour and 633 (42% of baseline) answered to the questionnaire on injuries and musculoskeletal health. Subsample of participants (n=371, 65% of baseline) took part in pre-participation screening with e.g. comprehensive medical examination by a physician, measurements of resting blood pressure, resting ECG, and fasting blood samples. Accelerometer measurements (one week wear-time) were included to those subjects taking part in pre-participation screening. Almost all the same measurements will be executed in the third wave data collection. In addition, for the first time national register data of the participants medication use and hospital discharge and operation registers will be combined with the data collected in this study.
The PA patterns of the participants will be identified by K-means cluster analysis (KmL) and advanced statistical analysis will be used in exploring determinants and health outcomes for each pattern.
This study identifies characteristics of different PA patterns – maintaining, decreasing and increasing activity – and different sports club participation patterns – maintainers, non-participators, drop-outs and sports club joiners – and thus give tools for evidence-informed and tailored health promotion. Moreover, the study clarifies whether it makes a difference to health if one maintains an active or inactive lifestyle, increases or decreases activity.
